The Recovery Support Navigator (RSN) are individuals holding a bachelor's degree in social work, psychology or related field who assist patients in Recovery in exploring treatment options, clarifying goals and strategies, provides education and resources and assists in accessing treatment or community resources.
The Recovery Support Navigator (RSN) will provide care management and system navigation supports to patients with substance use disorder (SUD) and will assist patients in achieving sustained recovery. The primary responsibility of RSN's is to support patients in exploring treatment options, clarifying goals and strategies, provides education and resources and assists in accessing treatment or community resources. The RSN should possess thorough understanding of community culture to address the barriers to successful recovery, better tailor health messages, and provide links to community resources and treatment options.
Outpatient based, initially meeting patients in a hospital setting and then transitioning with them to outpatient/community based care.
